Complex treatment of patients with combined trauma of the orbit

Abstract Details

Purpose:

to determine the methods and sequence of stages of medical care for patients with combined orbital injury.

Setting:

Tashkent Dental Institute

Methods:

The analysis of the treatment of 69 patients (12 to 64) from 2012 to 2017 Fractures of the orbit combined with contusion changes in the eyes of 39 patients with open injury of eyeball - 30. Diagnosis was carried out using Rg, CT, MTR.

Results:

primary reconstruction (1-14 days after injury) – 36 patients, delayed reconstruction (14-30 days) - 7 and late (4 months to 1.5 years) - 26.  To restore the anatomic structures of the orbit were used: titanium miniplates "Conmet", the Alloplant biomaterials, the plastic "Protakril"; tracheal system Ritenga. In 26 patients the reconstruction was carried out in 2 - 3 stages. The results were evaluated 6-8 months after the last operation, anthropometric measurements and photoregistration of facial images, calculation of the asymmetry index were performed. In all cases, a satisfactory result was obtained. The coefficient of asymmetry did not exceed 1.8.

Conclusions:

In case of combined trauma, the terms of surgical treatment are determined by the severity of TBI. When stabilizing the patient, the sequence is distributed: ophthalmologist, neurosurgeon, maxillofacial surgeon.
