Quality of life of patients with dry eye syndrome in the course of systemic lupus erythematosus

Abstract Details

Purpose:

Aim of this study is to present the assessment of quality of life connected with vision of patients with dry eye syndrome in the course of systemic lupus erythematosus. We evaluated  the frequency and diversity of the changes in the eyes in patients with systemic lupus erythematosus, with special consideration to pathologies related to the anterior segment of the eye.

Setting:

The study included 35 patients with SLE referred to Ophthalmic Outpatient of University Clinic in Katowice, Poland.The ophthalmological examination assessing anterior and posterior part of the eye with additional tests was performed. Based on the medical history and presented symptoms the most common ocular symptoms in SLE patients were analized.

Methods:

Every patient had the ophthalmological examination, which included: OSDI questionnaire, verification of visual acuity, slit light examination, corneal fluorescein staining with Oxford scale assessment, Schirmer test, TBUT and ophthalmoscopy. Evaluation of the subjective quality of life related to vision using the EQ-5D-5L questionnaire and analysis of the activity of SLE according to the SLEDAI scale in comparison with the ophthalmic indexes of the dysfunction of the anterior segment of the eyesight were performed.

Results:

The BCVA for the distance was on average  0.886 and 0.905 for the right and left eye in the group of the patients. We showed reduction in the total amount of water secreted in tears in patients using Schirmer Test (average - 11.34 mm / 5min), the break time of the tear film was TF-BUT 9.06, average OSDI was 31,99. The average negative correlation was (r = -0.3315) between the results of the EQ-VAS (Visual Analogue Scale) questionnaire and the results of the OSDI questionnaire (Ocular Surface Disease Index).

Conclusions:

The pathologies of the anterior segment of the eye are more common in patients with systemic lupus erythematosus compared to population. The degree of advancement of systemic lupus erythematosus affects the progression of pathologies related to the vision. Changes in the anterior segment of the eyes that occur in patients with systemic lupus erythematosus reduce the quality of life related to vision.
