Analysis of predictive factors of recurrence in Vogt-Koyanagi-Harada disease

Purpose:
To highlight parameters involved in the recurrence of Vogt-Koyanagi-Harada disease (VKH) to improve visual outcome
Setting:
Ibn Sina university hospital (hôpital des spécialités), Department A of ophthalmology, Rabat, Morocco
Methods:
In this retrospective study we reviewed the records of 54 patients attending the uveitis consultation over a period of 20 years. Patients with inflammatory recurrences and/or needing immunosuppressive therapy were classified as recurrent, whereas the other patients needing only steroïd treatment were classified as non-recurrent. The clinical and therapeutic features were analyzed using SPSS® software.
Results:
Our study included 59.3% women and 40.7% men. The mean age (years) was 31.09 ± 12.78. follow up duration (months) was 24.5 [8.25-42.75]. The non-recurrent group included 59.3% of patients versus 40.7% in the recurrent group. Initial visual acuity (VA) of the recurrent group was 1.8 LogMAR comparing to 1.3 in the non-recurrent group (p< 0.01). Using multivariate analysis, recurrence of VKH disease was associated with poor initial VA, longer duration of serous retinal detachment, later initiation of treatment, the need of immunosuppressive therapy and rapid tapering of steroïds (p< 0.05). Final VA of the recurrent group was 0.4 LogMAR
Conclusions:
Predictive factors of recurrence in VKH disease highlighted in this study could be useful to identify patients who need more careful follow up in order to initiate appropriate treatment and minimize the rate of recurrences.
