Thermal pulsations or conventional therapy for Meibomian gland dysfunction: how does treatment impact cataract surgery outcomes?

Abstract Details
Purpose:
To compare the current standard of care (Hot fomentation and topical antibiotic ointments) vs LipiFlow in the treatment of MGD and to analyse changes in tear film characteristics in patients undergoing treatment.
Secondary Objectives:
To study the impact of treating MGD before cataract surgery on patient satisfaction following the procedure.
To study the impact of tear film changes on corneal aberrations
Setting:
Tertary Eye Care Centre in North India ( Shroff Eye Centre)
Methods:
Prospective Randomized Single Centre interventional Hospital Study
Consecutive patients undergoing cataract surgery were recruited
Grading of MGD was performed by 3 independent observers.
Patients with Obstructive MGD (With presence of at least 1/3rd of Meibomian glands) were randomized to traditional treatment with hot fomentation and antibitic ointment (Group 2 , n=30) and LipiFlow (Group 1, n=30) using a random number generator
Patients with mild MGD were included in Group 3 ( no intervention arm,n=30). Objective criteria like Schirmers 1,2, TBUT, osmolarity, Meibo score,aberrations,Lipid layer thickness and SPEED score were studied in all the groups before and after teatment.
Results:
Schirmer 1 and 2 was significantly better in Group 1 compared to Group 2 ( p =0.003).
TBUT was significantly better in the Lipiflow group(9.66 +0.88) compared to conventional treatment ( 7.88 + 0.64) ( p=0.04). Meibo score, Abeerations and SPEED score were comparable between the two groups. Lipid layer thickness was significantly better i the Lipiflow group compared to the conventional treatment group ( p=0.002). Group 1 and 2 showed significant improvementt in parameters from the preoperative values after surgery
Conclusions:
Tretament of MGD before Cataract Surgery improves patient satisfaction after cataract surgery. Treatment with Lipiflow shows benefit over traditional therapy in patients with Moderate MGD. Further studies and a larger sample size are required to verify the same
