Intense pulsed light: a light to treat long-standing chalazia

Purpose:
To study the efficacy of Intense Pulsed Light (IPL) for treating recurrent and long-standing chalazia that failed to respond to first line medical care (topical antibiotics, topical steroids, and hot fomentation)
Setting:
: Nethradhama super-speciality eye hospital, Bangalore, India.
Methods:
In a prospective open-label study, 7 eyelids of 3 patients with long standing and recurrent chalazia were treated with IPL, using the M22 OPT (Lumenis). The periocular areas of eyelids were treated with IPL at 1-month intervals, until complete resolution or a maximum of 4 times (whichever occurred first). Outcomes, measured before each sitting, included the state of chalazion (complete resolution/ partial resolution/no improvement) and a score of dry eye symptoms (SPEED).
Results:
Average duration of chalazia before treatment was 15±7 months. All chalazia responded positively to IPL. Complete resolution was achieved in 1 of 7 (14%), 3 of 6 (50%) and 1 of 3 (33%) eyelids after 1, 2 and 3 IPL sittings, respectively. Partial resolution was achieved in 6 of 7 (84%), 3 of 6 (50%) and 2 of 3 (67%) eyelids after 1, 2 and 3 IPL sittings, respectively. Two eyelids with partial resolution are due for a 4th sitting.SPEED improved from 9.1±4 to 7.7±3.6,4.3±3.2, and 3.3±2.9 after 1, 2, and 3 IPL sittings, respectively
Conclusions:
IPL is a promising approach for treating long-standing chalazia that failed to respond to first line medical care, as a non-invasive alternative to surgical incision and curettage.IPL improved not only the eyelid lesions, but also dry eye symptoms. This correlation is not unexpected, since chalazia are caused by inflammation of tarsal meibomian glands.
