A prospective study of the outcome of a modified low-cost scleral fixated intraocular lens without the use of sutures or biological glue

Abstract Details

Purpose:

To study the visual outcome, implantation stability and complications of a modified low-cost scleral fixated Intra ocular lens (IOL) implantation in aphakic eyes.

Setting:

Retina Vitreous Service, L V Prasad Eye Institute, Bhubaneswar, India

Methods:

The outcome of a modified scleral fixation technique using commonly available instruments used in small incision cataract surgery was studied prospectively in 43 consecutive eyes rendered aphakic due to spontaneous/traumatic dislocation of lens/intraocular lens and iatrogenic causes.

Results:

The IOL tilt on ultrasound biomicroscopy (kappa 0.762; p<0.001) and the IOL centration (kappa 0.411; p=0.001) as assessed by two independent blind observers was satisfactory. The corrected distance visual acuity improved (pre-operative logMAR 0.5±0.54) significantly at 6weeks (logMAR 0.31±0.24; p=0.039) and at the mean final follow up (logMAR 0.22±0.24; p=0.0003) of 14.35±6.71wks. The residual spectacle correction in spherical equivalent was 0.74±1.2D. Post-operative vitreous hemorrhage was the most common complication (37.2%) which resolved spontaneously by 6wks.The cost of the surgery all-inclusive was less than 50 US dollars. The surgery was relatively fast (29±7mins).

Conclusions:

This method of trans scleral IOL fixation is stable, easily done and is possible to perform using low-cost instruments in a short surgical time.
