Correction of high myopia with astigmatism using topo-guided LASIK: 12-month results

Abstract Details

Purpose:

To evaluate the postoperative clinical results of the correction of high myopia with astigmatism using the topo-guided LASIK method.

Setting:

Astrakhan Clinical Hospital.

Methods:

A total of 136 eyes from 68 patients with myopia and astigmatism over -0.75 diopters (D), who had topo-guided LASIK, were included in this study. The average age of patients was 28 years. Preoperative mean spherical equivalent (SE) was -7.25 (range: -6.50 to -9.25 D) and refractive cylinder was -1.75 (range: -0.75 to -3.25 D). The personalized ablation was carried out using the software Contoura of the Allegretto WaveLight EX 500 excimer laser (Alcon), with a diameter of the optical zone of 6.5 mm. Thickness of corneal flap = 100 μm (mechanical microkeratome Moria Evolution 3E).

Results:

Analysis of diagnostic data performed on the 1st postoperative day, 1 month and 12 months after surgery showed that approximately 97.2% of eyes with a high degree of myopia with astigmatism achieved the same or better visual acuity, exceeding the maximum corrected visual acuity before operations, due to a decrease in the degree of astigmatism to the physiological and absence of higher order aberrations. In addition, 12 months after topo-guided LASIK, 98.8% of eyes after did not have vision loss, and 1.2% had a loss of only one line of best-corrected Snellen visual acuity.

Conclusions:

The use of the Contoura personalized ablation program allowed us to form a new profile of the cornea in patients in order to obtain consistently high visual acuity. The results of our study confirm the excellent efficacy, safety, good predictability and stability of the correction of high myopia with astigmatism using the topo-guided LASIK method.
