Intraoperative complications during small-incision lenticule extraction (SMILE)

Purpose:
To evaluate the intraoperative complications of SMILE for myopia and myopic astigmatism correction
Setting:
Selcuk University Faculty of Medicine, Department of Ophthalmology, Konya, Turkey
Methods:
Patients who had small-incision lenticule extraction surgery were examined 1 day, 1 week, 1 month, 3 months, 6 months, 12 months postoperatively. Demographic characteristics, visual outcomes, and intraoperative complications were recorded. Thus, the incidence and management of the complications and the potential factors were analyzed.
Results:
A total of 156 eyes (81 patients) were assessed, 36 (23,07%) of eyes had intraoperative complications, including suction loss (2 eyes [1,28%]), cap rupture (1 eye [0,64%]), opaque bubble layer (20 eyes [12,8%]), subconjunctival hemorrhage (3 eyes [1,9%]), black spots (3 eyes [1,9%]), unintended posterior plane dissection (3 eyes [1,9%]), tearing of the lenticule (1 eye [0,64%]), epithelial defect (5 eyes [3,2%]), partially retained lenticule (1 eyes [0,64 %]), and inaccurate laser pulse placement due to eye movement (1 eyes [0,64%]). The complications were resolved using appropriate management procedures; all cases had satisfactory visual outcomes.
Conclusions:
Although intraoperative SMILE complications may occur, most of them are related to inexperience. Immediate recognition and appropriate management is fundamental for optimizing the final visual results.
