Outcome of transepithelial photorefractive keratectomy (PRK) for extreme myopia with high-speed excimer laser and advanced laser beam profile

Abstract Details

Purpose:

To evaluate 12-month postoperative outcomes of extreme myopic eyes with transepithelial photorefractive keratectomy (TE-PRK) using SCHWIND AMARIS (SA) 1050 with an upgraded laser beam profile.

Setting:

Laser Eye Centre

Methods:

Retrospective case series.  The outcomes of extreme myopic TE-PRK treatments performed with the SA1050 excimer laser with SmartSurfACE beam profile were evaluated at 12 months postoperatively. At completion of ablation Mitomycin C 0.02% applied. Refractive and visual comparisons of manifest refraction, uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), and safety (change of CDVA from baseline).

Results:

A total of 122 eyes with a mean preoperative spherical equivalent refraction of -11.79±1.54D underwent SmartSurfACE PRK. The achieved treatment effect ranged from -10.1D to -16.9 D (mean -12.1D). 106 of 122 (85%) showed UDVA ≥20/40 postoperatively.  114 (93%) had no change or improved CDVA, 32 (26%) had improved CDVA 1 line or more, 6 (5%) gained 2 lines or more while 2 (2%) lost 2 lines or more. 4 eyes had visually significant haze. No re-treatments performed.

Conclusions:

Trans-epithelial photorefractive keratectomy with Schwind Amaris SmartSurfACE excimer laser achieved good safety and efficacy in eyes with extreme myopia (≥ -10 D).  85% achieved ≥20/40 UDVA postoperatively. 93% no change or improved in CDVA and 5% gained 2 lines or more. TE-PRK with a high-speed excimer laser procedure may be a good option to consider for the correction of extreme myopia.
