The role of new ablation algorithm (SmartSurfACE) in clinical and functional outcome of myopic patients with one-step TransPRK approach

Abstract Details

Purpose:

Surface ablations, according to literature and our own data, are safe and effective. Nevertheless the slow vision recovery and pain syndrome are decreasing the acceptance of the method in between the patients and surgeons. New ablation algorithm gave us promising results, which are solving these last questions.

Setting:

Laser Surgery Clinic "Sphere", Moscow, Russian Federation

Methods:

Two groups of 50 myopic patients -0,5 to -8,5 D and astigmatism up to 3,5 D were compared. Trans-epithelial keratectomy (TransPRK) with standard aberration free algorithm (retrospective analysis) in one group and with SmartSurfACE algorithm in other group were performed, using the SCHWIND AMARIS 500E excimer laser. Preoperatively there were no significant difference between both groups found. Ablation was planned with SCHWIND CAM software (all SCHWIND eye-tech-solutions) with an average OZ 6,98±0,51mm and depth 137± 27µm. Silicon-hydrogel contact lens and standard medication were used postop. Patients were observed at 1,4, days and 1, 3, 6 months postop

Results:

UCVA in SmartSurfACE group at these periods 0,54±0,22; 0,56±0,17 and 0,97±0,1; 1,04±0,13; 1,06±0,14.Objective refraction at 1,3,6 months postop: 0,01±0,19; -0,01±0,16,  0,03±0,14D. No one patient lost any line of BCVA, 30% of them had improved, 96% of patients were within ±0,25D from the target refraction.Pain at 1-st day postop was graded from absence to moderate and in average classified as “low”. The tear production tests (Schirmer Test1 and TBUT preop and at 3 and 6 months postop) didn’t change within the observation time in comparison to preoperative.

Conclusions:

We found a statistically significant improvement in SmartSurfACE group for UDVA recovery 1d-1m-3m-6-m postop, in accuracy 3 and 6 months postop, spherical aberration value 6 months postop. New ablation algorithm provided us a faster vision recovery, excellent accuracy and safety. Preserving the tearfilm stability. Thus we have more arguments in favor of surface ablations to correct myopia up to -8,5 D
