Authentic exfoliation syndrome (AES) or capsular delamination: regarding one case

Abstract Details

Purpose:

The AES is an infrequent disease. Its incidence and prevalence are unknown. The diagnosis is usually incidental, since it is asymptomatic. It consists on the delamination of the anterior capsule of the lens. The risk factors associated with this entity are trauma, inflammation and frequent exposure to heat and/or infrared radiation.

Setting:

Hospital de l'Esperança - Parc de Salut Mar (Barcelona, Spain).

Methods:

A 91-year-old woman presented with a slowly progressive loss of visual acuity (VA) in both eyes (BE). As an ophthalmological background, she presented peripheral iridotomies performed with YAG laser. The best corrected visual acuity was 0.3 in the right eye (RE) and 0.4 in the left eye (LE). She presented corticonuclear cataracts and a raised filamentous flap on the anterior capsule in BE.  An anterior segment oct was performed in order to determine the extent of this alteration exfoliation to perform an optimal planning of cataract surgery.

Results:

AES has been associated with professions with great exposure to heat, and is considered an effect of IR radiation on the equatorial cells of the lens. However, the series presented by Teekhasaenee et al, it was observed that 248 of the 259 cases were idiopathic. The average age at diagnosis is 70 years. It is asymptomatic and the diagnosis is clinical, although it can be supported by imaging techniques as  anterior segment OCT.It does not need treatment.During cataract surgery, it is recommended to use trypan blue and perform a wide capsulorhexis.It is associated with a 10% risk of lens dislocation.

Conclusions:

The AES is a rare and asymptomatic entity that does not modify the clinical management of the patients who present it. Surgically, it must be taken into account to prevent intraoperative complications. The OCT SA constitutes an innovative tool for its diagnosis.
