Purpose:
To evaluate the utility of HD-Anterior Segment Optical Coherence Tomography (AS-OCT) in the classification and management of herpes simplex (HSV) keratitis.
Setting:
Corneal department. Hospital Universitari Vall d'Hebron. Barcelona. Spain.
Methods:
HSV keratitis has been classically classified according to the affected corneal layer based on clinical symptoms and slit lamp examination.
Epithelial, stromal and endothelial keratitis may be presented clinically in a similar way.
AS-OCT has been used as an important tool for the diagnosis of multiple corneal disorders as it allows for an in vivo visualization of all corneal layers.
For the purpose of this study, cross sectional Swept Source HD AS-OCT images were performed in patients with clinical diagnosis of HSV keratitis in order to determine the layer affected and the kind of involvement (edema, infiltrate, necrosis or fibrosis).
Results:
Patients with clinical diagnosis of epithelial keratitis showed subephitelial stromal infiltrate in AS-OCT. The endothelial involvement was evidenced by increased pachymetry, hyperreflective endothelium and presence of keratic precipitates. An important number of patients initially diagnosed as only having stromal keratitis, presented on the AS-OCT also endothelial and/or epithelial invovement.
Conclusions:
AS-OCT provides an in vivo, non-invasive and rapid manner to ellucidate the depth of corneal involvement and it turns out to be a useful tool to classify HSV keratitis ans therefore, to help make therapeutic decisions.
