Combined phacoemulsification with ExPress implantation for patients with cataract and refractory glaucoma

Abstract Details
Purpose:
To evaluate the safety and efficacy of phacoemulsification and exPress implantation for patients with cataract and refractory glaucoma.
Setting:
Retrospective interventional case series.
Methods:
In this non-comparative, retrospective study, we included 24 consecutive Chinese patients (27 eyes) who underwent combined phacoemulsification with exPress shunt by a single surgeon from September-2016 to Agust-2018 were included. The main outcomes were visual acuity (VA), intraocular pressure (IOP), antiglaucoma medication (AGM), complications and reoperations.
Results:
The mean follow-up was 13.6±9.2 months. The postsurgical VA improved in 13 eyes (48.1%). The mean IOPs were 35.3 ± 11.5 mmHg at baseline, 15.2 ± 8.7 mmHg at post-surgery and 17.9±6.3 mmHg at final follow-up (P<0.001), respectively. The mean numbers of AGM were 3.4±1.1 at baseline and 1.1±0.4 at the last follow-up (P<0.001). There were 18 eyes (66.7%) as a complete success, and 5 eyes (17.9%) as a qualified success, The most common complication was a hypertensive phase in 8 eyes (29.6%). 4 eyes underwent further surgery due to uncontrolled IOP.
Conclusions:
Combined phacoemulsification and exPress implantation is an effective and safe procedure, providing good control of IOP for patients with refractory glaucoma and cataract.
