CO2 laser-assisted deep sclerectomy surgery (CLASS) in the treatment of open-angle glaucoma in Chinese patients

Purpose:
To evaluate the efficacy and safety of CO2-laser assisted sclerectomy surgery (CLASS) with 5-fluorouracil (5-FU) for open-angle glaucoma in the Chinese population.
Setting:
Study of the efficacy and safety of CLASS surgery in Chinese glaucoma patients.
Methods:
This was a retrospective, uncontrolled, interventional case series. All patients from 2016 to 2017 who received CLASS were recruited in this study. The primary outcome was the change in intraocular pressure (IOP) and the number of IOP-lowering medications over a 12-month follow-up period. Adverse events were evaluated as secondary outcomes. Wilcoxon rank-sum test was used for statistical analysis.
Results:
The average preoperative IOP was 31.3±7.6 mmHg. During 12-month follow-up period, the mean percentage of IOP reduction from baseline was 48% (P<0.01). The mean percentage of IOP reduction from baseline at post-operative months (POM) 1, 3, 6, 9 and 12 were 48.1% ± 24.6% (95% CI, 40.4-55.7), 51.4% ± 19.3% (95% CI, 45.4-57.4), 51.2%±17.2% (95% CI, 45.9-56.6), 50.9% ± 15.0% (95% CI, 46.3-55.6), 49.2% ± 16.3% (95% CI,50.0-54.3). The average number of glaucoma medications decreased from 3.02±0.81 at pre-operation to 0.05±0.22, 0.10±0.37, 0.12±0.40, 0.17±0.44 and 0.24±0.58 at POM 1, 3, 6, 9 and 12, respectively.
Conclusions:
The CLASS procedure with 5-FU is a simple procedure with excellent safety and efficacy for reducing IOP and the number of IOP-lowering medications with a mean follow-up of 12 months. Hence, it could be a promising treatment option for open-angle glaucoma (OAG) patients.
