Glaucoma surgery and induced astigmatism: hypotony and blepharoptosis influence

Abstract Details

Purpose:

To report how hypotony and aponeurotic blepharoptosis may modify reversible induce astigmatism after glaucoma surgery.

Setting:

Private practice clinic.

Methods:

We present the case of a 65-year-old woman with acquired aponeurotic ptosis and pseudo-exfoliative glaucoma in her right eye. She underwent clear corneal phacoemulsification and Express (P50 version, Alcon Inc., Fort Worth, TX) shunt surgery with mitomycin-C in the right eye. Before surgery, intraocular pressure (IOP) on maximal topical drugs was 35 mmHg and corneal astigmatism -0.5 at 147º. Two months after surgery, IOP without medication was 8 mmHg and her visual acuity was 20/20 with -2.4 at 170º corneal astigmatism.

Results:

Due to corneal topographic changes in our patient after glaucoma surgery, we decided to treat hypotony by adding topical dexamethasone twice a day. Two weeks after, right eye IOP was 16mmHg and corneal astigmatism recovered previous values (-0.5 150º). Corneal astigmatism remained stable six months later.

Conclusions:

Corneal curvature changes after glaucoma surgery may not only be due to overhanging filtering bleb but also to eyelid alterations associated with hypotony.
