Pattern electroretinography (pERG) in the evaluation of a glaucoma suspect with high myopia: case report

Purpose:
To report a case in which the pERG assisted the investigation of a glaucoma suspect with high myopia.
Setting:
Hospital de Olhos do Paraná, Curitiba/PR, Brazil.
Methods:
Case report of a patient referred to the glaucoma department for glaucoma suspicion. It was performed complete ophthalmologic evaluation including slit-lamp examination, fundus examination, gonioscopic examination, photography, visual field test, optical coherence tomography (OCT) and pERG (Diopsys® PERG, Diopsys Inc, Pine Brook, NJ, USA).
Results:
Female, 63-year-old, previous high myopia (OD: -18.00D; OS: -21.00D), phacoemulsification performed 16 years ago in both eyes (OU). Mother has glaucoma. BCVA OD: 20/20; OS: 20/30. Gonioscopy OU: open angle. Eye fundus examination OU: extensive peripapillary atrophy and tilted optical disc, making it difficult to delimitate the cupping (apparently small). Intraocular pressure OD: 15 mmHg; OS: 13 mmHg. OCT OU: areas of significant peripapillary thinning, inconclusive due to high myopia. Visual Field: OD: scotoma close to blind spot and lower nasal. OS: scotoma in temporal region, greater superior, near blind spot. pERG: normal. It was decided not to start treatment.
Conclusions:
The reported case demonstrates the difficulty in assigning alterations of complementary exams to the glaucoma diagnosis in a high myopia case. The use of new tools as pERG can help with new information and support decisions. In the reported case, the normal pERG result, which indicates no signs of ganglion cell suffering, led us to choose not to start treatment. However, follow-up with periodic and serial examinations will guide on the treatment decision.
