New combined-technique phacoemulsification with modified tunnel trabeculopuncture in pseudoexfoliative glaucoma

Abstract Details

Purpose:

to estimate efficacy of combined phacoemulsification with modified tunnel trabeculopuncture (MTTP) in patients with pseudoexfoliative  glaucoma.

Setting:

Methods:

Combined phacoemulsification with modified tunnel trabeculopuncture was performed on 275 eyes with pseudoexfoliative  glaucoma (initial stage). Incision was made in Schlemm`s canal from inside lateraly to filtrating zone with the anterior lens capsule insertion in this trabeculopuncture as a stent.  In 5 years following up period intraocular pressure, computer perimetry and  optic nerve OCT were assessed.

Results:

Average IOP level in patients before operation was 27,5±0,34 mmHg. In one month after surgery it rose to 33,6±0,74 mmHg, with the next dropping to 14,1±0,65 mmHg in three months. From 6 months till 5 years after surgery it was changed insignificantly from 15±0,89 tol 17,8±0,51 mmHg. OCT data and light sensitivity were stable from 72,34 mkm till 64,31 mkm and from -4,06 dB till -4,62 dB respectively.

Conclusions:

Combined phacoemulsification with modified trabeculopuncture is effective surgery technique, that decrease IOP by 35%, MD in 15%, average thickness of optic nerve fibres in 11% with pseudoexfoliative glaucoma stabilization for five years.
