Comparison of the astigmatic outcomes of toric IOL implantation using femtosecond laser-assisted capsular marks vs intrastromal corneal marks combined with intraoperative aberrometry

Abstract Details

Purpose:

Comparison of the astigmatic outcomes of toric IOL implantation using femtosecond laser assisted capsular marks versus intrastromal corneal marks combined with intraoperative aberrometry.

Setting:

Lindenhurst Ambulatory Surgery Center, Lindenhurst Illinois USA

Methods:

This retrospective study included 76 eyes of 38 patients with cataract and pre-existing astigmatism that had undergone toric IOL implantation (TECNIS Symfony® Toric Extended Depth of Focus IOL) using femtosecond laser assisted cataract surgery. In group A (n=26), toric IOLs were aligned using iris registration-guided capsular marks (CMs) with automatic cyclotorsion compensation (IntelliAxis-L, LENSAR). With this technique, there is negligible concern for parallax as the CMs are very close to the coronal plane of the IOL. In group B (n=50), toric IOLs were aligned using intrastromal corneal marks (IntelliAxis-C, LENSAR) and intraoperative aberrometry (ORA™, Alcon).

Results:

Data are being collected. Preliminary results show good astigmatic outcomes obtained when aligned using CMs. Statistical analysis using paired t test has revealed better uncorrected visual acuities at distance, intermediate, and near vision OU in group A over group B when using capsular marks for steep axis toric IOL alignment compared with intrastromal corneal marks with LENSAR femtosecond laser system.  No complications have been observed till date.

Conclusions:

Femtosecond laser-assisted capsular marks facilitate precise alignment of toric IOL on the predefined axis of implantation, thus allowing for effective management of astigmatism with toric IOLs.  The need to eliminate the need for pseudophakic axis confirmation with ORA can be eliminated and time-saving intraoperatively with use of capsular marks alone.
