Study on corneal surface changes induced by intraoperative gaze in SMILE and intervention with different methods

Abstract Details

Purpose:

To observe the changes of corneal anterior surface smoothness of  non-surgical eyes due to corneal exposure during SMILE, and to explore the comparative study of different intervention methods.

Setting:

Chongqing Ophthalmological Hospital Chongqing China

Methods:

3 groups: eyeshade group, gel group and routine group (20 patients in each group). Before SMILE was performed on the right eye, the left eyes of patients in the eyeshade group were covered, while gel was applied to those in the gel group. The routine group was not treated. The left eye corneal topographic map and slit lamp examination were repeated 5, 10 and 15 minutes later. Finally, the corneal anterior surface curvature, corneal thickness, SAI, SRI and IAI at each point-in-time were observed and compared.

Results:

The left eye showed that there was no significant change in SAI, SRI, IAI, corneal anterior surface curvature and corneal thickness in the gel group compared with preoperative examination. However, SAI, SRI and IAI in the eyeshade group and the routine group had significant changes, while corneal anterior surface curvature and corneal thickness had changes, but there was no statistical significance. There were some differences in SAI, SRI and IAI between the eyeshade group and routine group, but there was no statistical significance. However, the indexes of the eyeshade group and the routine group gradually returned to normal after closed-eye rest.

Conclusions:

Corneal exposure during SMILE can lead to changes in the anterior corneal surface and increase irregularity, which may ultimately affect the subsequent surgery and postoperative outcomes. Gel application can effectively prevent the increase of corneal surface irregularities.
