Double SMILE in an unusual situation

Abstract Details

Purpose:

To present a clinical case of double Smile in cases of an untouched area of the cornea.

Setting:

Irkutsk Branch of S. Fyodorov Eye Microsurgery Federal State Institution, Russian Federation.

Methods:

Patient with moderate myopia in both eyes is scheduled for Smile surgery using standard technology in fast mode (corneal flap 130 μm and diameter 7.8 mm; corneal access width 2.38 mm, OZ 7.0 mm, minimum thickness 15 μm). During surgery due to gradual appearance of milky-colored opacification in interface, both corneal flap and lenticular remained untouched from 5 to 7 o'clock. Standardly, such a complication requires transfer of surgery to later date. In our case, after removal of treatment pac, corneal edema wasn’t detected and centering didn’t cause difficulties, it was decided to repeat Smile with same parameters.

Results:

Intra- and the postoperative periods were uneventful. Visual acuity after surgery on the first day was 0.9; refraction sph +0.25 cyl - 0.5 ax 154. After 2 weeks visual acuity 1.0 with refraction sph + 0.25 cyl - 0.52 ax 152. According to Pentacam, the optical zone with clearly defined boundaries is topographically homogeneous; According to the OCT data cornea inclusions, remnants of lenticules, corneal valve dissection or a bed were not detected.

Conclusions:

The absence of corneal edema and the rapid restoration of corneal transparency allowed us to re-establish treatment pac and to fulfill secondary Smile operation obtaining a high functional result. The restoration of the patient’s vision in a planned manner provides a positive attitude towards the Smile method and the surgeon.
