Purpose:
To compare the demarcation line depth and efficacy after accelerated corneal crosslinking (CXL) for progressive keratoconus using dextran-based and hydroxypropyl methylcellulose (HPMC)-based riboflavin solutions.
Setting:
Prospective study
Methods:
In a comparative case series, CXL was performed in 39 patients (39 eyes) using a riboflavin solution containing either dextran-based riboflavin (n = 18) or HPMC-based riboflavin (n = 21). Changes in visual acuity (VA), refraction, corneal thickness in thinnest location (TCT), maximal keratometry reading (Kmax), and endothelial cell density (ECD) were recorded. Stromal changes one month after surgery were analysed using optical coherence tomography (OCT).
Results:
The demarcation line depth was 286.77 ± 53.97 μm in the dextran-based riboflavin group and 357.80 ± 23.54 in the HPMC-based riboflavin group (p < 0.001). No statistically significant differences were found in the change in VA, TCT and Kmax between the groups after 6 months. Endothelial cell density (ECD) was stable in both groups.
Conclusions:
We found deeper structural changes in the HPMC-based riboflavin group than in the dextran-based riboflavin group, although both solutions were safe for the endothelium at 6 months.
