Long-term results of collagen cross-linking (CXL) for progressive keratoconus in paediatric age group beyond eight years of follow-up

Abstract Details
Purpose:
To study the response of keratoconic eyes in pediatric age group (less than 18 years of age) to CXL after a minimum follow up of 8 years.
Setting:
Department of Cornea & External Disease in a tertiary eye care center
Methods:
Children less than 18 years of age with progressive keratoconus had CXL – standard protocol (thinnest pachy greater than 450µ) or hypotonic CXL (thinnest pachy less than450 µ). The induced changes in visual acuity, refraction, topographic and tomographic indices were analysed at a minimum follow up of 8 years.
Results:
58 eyes of 46 patients, mean age15.26±2.18 years had CXL. 41eyes had standard CXL, 17 had hypo-osmolar CXL. After a mean followup of 8.89±0.70 yrs (8.02-11.24yrs), there was improvement in CDVA by 0.07±0.04 logMAR CI(-0.003, 0.16), significant reduction in Topographic cylinder by 1.84±2.4D CI(1.13, 2.54), and in Kmax by 1.79±4.92D, CI(0.29, 3.29) reduction in Kmax was more in moderate Keratoconus. There was significant thinning in Thinnest pachymetry by 43.8±45.8µ at last follow up. Stabilization/flattening of Kmax was seen in 78% eyes, there was no difference in the results between isotonic(78%) and hypotonic CXL (77%)
Conclusions:
CXL shows good results in the pediatric age group, achieving long term stabilization of the disease in a majority of patients even after 8years of follow up
