Corneal perforation after transepithelial (Epi-On) accelerated cross-linking

Abstract Details

Purpose:

To present a case of corneal perforation after Trans-epithelial (Epi-On) Accelerated Cross-linking. Safety parameters, technique and patient selection were correctly considered before CXL treatment. As well as to share the postoperative management and outcomes of this case.

Setting:

Montevideo

Methods:

A 28 year old male with Down syndrome presents with corneal perforation in Crosslinking Post-op Day 20. Penetrating keratoplasty was first done but failed do to corneal melting secondary to floppy eyelid. Second penetrating keratoplasty was performed succesfully with lateral tarsorraphy.

Results:

Penetrating keratoplasty 4th month follow op presented with closed lateral tarsorraphy, clear cornea and no signs of graft rejection or corneal melting.

Conclusions:

Besides correct pre-operative patient selection for Trans-epithelial (Epi-On) Accelerated Cross-linking, corneal perforation can occur. In Down syndrome patients one should consider to perform a lateral tarsorraphy with penetrating keratoplasty to prevent corneal graft from melting.
