Iontophoretic corneal collagen cross-linking in patients with pellucid marginal degeneration: one-year results

Purpose:
To report the results of iontophoretic corneal collagen crosslinking (I-CXL) with riboflavin and ultraviolet A irradiation in patients with pellucid marginal degeneration (PMD).
Setting:
Oftaprof Clinic, Iasi, Romania
Methods:
Fourteen eyes from 8 patients with progressive PMD were treated by I-CXL. The treatment was slightly decentered inferiorly, preserving an untreated surface area 1 mm from the superior limbus. The follow-up period was 12 months for each case.
Results:
Mean preoperative logMar uncorrected visual acuity (UDVA) was 0.72 (SE=0.04), and mean preoperative logMar corrected distance visual acuity (CDVA) was 0.32 (SE=0.06).
Twelve months postoperatively, mean UDVA was 0.59 (SE=0.06) and mean CDVA was 0.22 (SE = 0.02).The improvement of UDVA and CDVA was statistically significant (p< 0.05, Wilcoxon test).Mean preoperative K min was 40.98 ± 4.85D and significantly decreased to 40.38 ± 2.44 D, 12 months postoperatively (p = 0.04).Also, mean preoperative Kmax was 54.35 ± 4.37 D and significantly decreased to 52.22 ± 2.76D,12 months postoperatively (p = 0.003). No side effects were noticed during the follow-up period.
Conclusions:
Iontophoretic corneal collagen crosslinking proved to be a safe and efficient method for halting the progression of pellucid marginal degeneration, with improvement of visual acuity and reduction of keratometry.Studies with larger sample size and longer follow up are needed .
