Keratoconus in paediatric age group: is collagen cross-linking the way to go?

Purpose:
To present the results of safety and efficacy of corneal collagen cross-linking (CXL) in the treatment of keratoconus in paediatric patients
Setting:
Tertiary eye care center
Methods:
This is a retrospective, observational case series of 50 eyes of 44 patients (aged 16 years or younger) with progressive keratoconus who underwent CXL by any of these methods and had completed at least 6 months of follow up - Standard epithelium-off CXL was performed in 31 eyes, contact lens assisted CXL in 16 eyes and transepithelial CXL in 3 eyes. Principal outcomes included corrected distance visual acuity (CDVA), mean refractive spherical equivalent (MRSE), and topography-derived outcomes of maximum and average keratometry (K).
Results:
37 of 50 eyes showed stabilisation of disease and 10 eyes showed regression. Mean CDVA from 0.65 ± 0.22 to 0.35 ± 0.15 logMAR (P=.003). Mean change in maximum Keratometry was noted to be significant (1.01 ± 2.40 diopters) (P=.004). MRSE remained stable (preoperative MRSE = -4.29 D, SD = 4.04, and postoperative MRSE = -4.53 D, SD = 4.07, P = 0.31). No significant complications were noted.
Conclusions:
Corneal collagen cross-linking is an effective treatment option for progressive keratoconus in paediatric patients. The improvement in terms of topographic and visual outcomes is comparable to the results seen in adult population.
