Correlation between keratoconus progression and demarcation line depth in epithelium-off and trans-epithelium corneal cross-linking

Abstract Details
Purpose:
To compare corneal demarcation line depth in both accelerated epithelium-off corneal cross linking (Epi-off CXL) and trans-epithelium corneal cross linking (TE-CXL) using Anterior Segment Optical Coherence Tomography (AS-OCT) and its relation to keratoconus progression in both techniques.

Prospective comparative study
Methods:
Patients with mild to moderate progressive keratoconus were included and classified into two groups; Group A with corneal pachymetry more than 400µm were assigned to have accelerated Epi-off CXL and Group B with corneal pachymetry less than 400µm were assigned to have accelerated TE-CXL. Assessment of corneal demarcation line depth (DL) was carried out after 3 months by AS-OCT. Maximum keratometry readings and corneal pachymetry changes were evaluated after one year follow up using the Scheimpflug imaging system (OCULUS Pentacam).
Results:
54 eyes of 34 patients were included (31 Group A & 23 Group B) . At 3 months follow up, mean corneal DL depth in Group A was 219.87 ± 58.36 µm that represented a mean percentage of 45.5% of the mean preoperative pachymetry while in Group B was 127.15 ± 7.73 µm with percentage of 32.2% (p-value<0.05).At one year follow up, the mean Kmax change was - 0.5 ± 0.3D in Group A and was 0.4 ± 0.2 D in Group B while mean pachymetry change was 6.4 ± 4.7 &-10.1 ± 2.3 µm in both groups respectively (p-value>0.05).
Conclusions:
Despite a significantly deeper corneal demarcation line depth created by accelerated Epi-off CXL technique compared to accelerated trans-epithelium CXL; there was no significant impact on keratoconus progression.
