Abstract Details
Purpose:
To audit the quality of cornea transplant tissue supplied and to study the indication, types, risk factors and visual outcome of corneal transplant surgery performed in our hospital over a three years period between January 2015 to December 2017.
Setting:
Sheikh Khalifa Medica City. A tertiary referral center for corneal transplant.
Methods:
Electronic medical records (Cerner system) of all patients who had had corneal transplant surgery between January 2015 and December 2017 were reviewed. Data pertaining to above mentioned donors and recipients related elements of cornea transplant procedure were collected and analysed using SPSS package.
Results:
117 patients were identified and the most common indications for corneal transplant were; corneal melt and infection, corneal scar and keratoconus. 74% of surgeries were penetrating or anterior lamellar keratoplasty and 15% were posterior lamellar transplant . Risk factors for graft rejection includes; preoperative inflammation (26%), corneal neovascularisation (33% ), raised intraocular pressure (31% ). Mean visual acuity improve from 0.14 to 0.38 (P=0.00) . Median endothelial cell density of donors was 2915 cells /mm2, median age of donors was 44 years, median death to transplantation time was 8 days and the incidence of positive donor microbial culture was 4.5%.
Conclusions:
The audit revealed a highly significant improvement of vision following corneal transplant surgery despite high proportion of risk factors among receipients. The number of penetrating keratoplasty was much higher than lamellar due to full thickness pathologies and least number of posterior lamellar transplant due to rarity of endothelial diseases in the region. The donor tissue supplied to our hospital was of good quality in terms of tissue viability and transportation time. The lack of specular microscopy machine hinders long term assessment of the viability of transplanted tissue.
