Hot PKP in infectious keratitis: timing, visual outcome and factors of success

Purpose:
To evaluate early PKP to eradicate infectious process, remove the main bulk of infection and restore anatomical and functional outcome in treatment of resistant progressive infective keratitis
Setting:
Memorial institute for ophthalmic research ,Egypt
Methods:
Our study were done on 20 eyes with resistant progressive infective keratitis due to different causes not responding to aggressive medical treatment underwent therapeutic PKP and followed up for 8-12 months .All patients were done by same method and in one of them the lens were removed .
Results:
Clear graft were obtained postoperatively in 14 out of 20 eyes, late recurrence occur in one case after 2 months and respond to treatment, in 3 cases integrity of globe is restored but graft is not clear and progression occur in 2 cases .BCVA in 8 cases were between o.2 and o.5, in 7 eyes was between HM and 0.01 due to cataract and graft opacity and deterioration occur in 2 eyes.
Conclusions:
Our results shows that PKP for progressive keratitis is successful treatment for eradicating infection ,restoring anatomical integrity of the eye and visual rehabilitation in most eyes .Early therapeutic PKP offers better results and visual outcome .
