Fine needle diathermy of corneal neovascularization prior to refractive lens exchange in a high hyperope: a case report

Purpose:
To present a case where fine needle diathermy successfully treated corneal neovascularization associated with lipid keratopathy prior to refractive lens exchange in patient with high hyperopia
Setting:
Section of Cornea, External Diseases and Refractive Surgery, Eye and Vision Institute, The Medical City, Ortigas Avenue, Pasig City, Metro Manila, Philippines
Methods:
This is a case report. Literature search was done using the PubMed database.
Results:
A 56-year-old male with chronic contact lens use for high hyperopia (manifest refraction: right+10.00, left+14.00()5.75x135) presented with a gradually enlarging corneal opacity on the left eye. Exam revealed a dense stromal opacity with lipid exudation (3.0mmx4.0mm) at the paracentral area connected to a large corneal vessel complex. Fine needle diathermy was performed to occlude the feeder vessels. After three months, total resolution of the vessels with no increase in the size and density of the corneal lesion was observed. Best-corrected visual acuity (BCVA) remained stable. Refractive lens exchange was done on both eyes. BCVA at 1 month was logMAR 0.16.
Conclusions:
Fine needle diathermy can be used to effectively treat deep corneal vascularization associated with lipid keratopathy from chronic contact lens use. Timely occlusion of the feeder vessel complex prevented extension of the lesion over the visual axis, for which penetrating keratoplasty may have been warranted. Refractive lens exchange was performed successfully yielding a good visual outcome in this case.
