Keratoconus treatment with intrastromal corneal segments complicated by epithelial ingrowth and an ulcerative keratitis in long-term post-op period: case report

Abstract Details

Purpose:

To report clinical case about patient who underwent  treatment of keratoconus by implantation of corneal intrastromal segments with difference in time is about 1 year between them.

Setting:

Refractive surgery department, eye clinic “ Sokol”, Rostov-on-Don, Russia.

Methods:

Implantation of corneal segment and postoperative period was unremarkable in right eye. Both intrastromal tunnels were created with femtosecond laser FS200 Alcon. Postoperative period of left eye was complicated by repeated keratitis and formation of haziness within notch area. An epithelium ingrowth process into corneal tunnel was defined with OCT. On 7th month the patient complained of strong pain appearance in the left eye. He noted the emergence of severe pain after waking up when his arm  accidentally contacts with the eye surface. An ulcerative keratitis within notch area with extrusing piece of segment was diagnosed, the segment was explanted.

Results:

UCVA of right eye remains is equal to 20/20 until current time. UCVA of left eye decreased and stabilized up to 20/50. Currently (6 month after explantation) UCVA is unchanged. The refractive error post-explantation returned to a level approximating that of the preoperative refraction.

Conclusions:

Extrusion of segment was provoked by a combination of an epithelium ingrowth process and directly by the factor of mechanical injury as well as the state of the eye surface  (the presence of severe dry eye syndrome as a concomitant factor).
