Clinical characteristics of patients with ocular neuropathic pain

Abstract Details

Purpose:

To analyze the clinical characteristics of ocular neuropathic pain and its association with the status of dry eye and patient’s personal history.

Setting:

Department of Ophthalmology, Chonnam National University Medical School and Hospital

Methods:

This study evaluated 26 patients with neuropathic ocular pain from January 2018 to November 2018. Detailed history on the ocular surface disease, surgery, and systemic disease was obtained. Tear break-up time (TBUT), Schirmer score, and corneal staining score were measured, and ocular pain assessment survey (OPAS) score was analyzed. Patients were divided into two groups according to the type of dry eye (aqueous deficient type dry eye [ADDE], n=13; non-ADDE, n=13), the severity of pain (severe, n=14; mild-to-moderate; n=10), and presence of systemic disease (presence, n=14; absence, n=12), and comparative analysis were performed between the groups.

Results:

The mean age of the population was 57.9 ±12.9 years and 65.4% (n=26) of patients were female. Five patients underwent cataract surgery. The mean TBUT, Schirmer score, and corneal staining score were 4.08 ± 1.44 sec, 6.42 ± 2.79 mm, and 0.58 ± 0.86. There was a significant aggravation in photosensitivity-related pain and impairment of reading in the ADDE group compared with the non-ADDE group (P=0.01 and P=0.04). Declining of walking, reading, and mood was more severe in the group with severe pain and the group with systemic diseases (all P<0.05).

Conclusions:

The clinical characteristics of patients with ocular neuropathic pain could be different according to the type of dry eye, intensity of pain, and patient’s history, and should be considered in diagnosis.
