The accuracy of centre-wise Pentacam pachymetric indices in the diagnosis of forme fruste keratoconus

Abstract Details

Purpose:

To offer a novel center-wise pachymetric index for diagnosing forme fruste keratoconus.

Setting:

Salouti Eye Clinic; Shiraz, Iran.

Methods:

A total number of 4947 eyes from 4947 patients, which were evaluated with Pentacam HR, were enrolled and used for developing the indices. Of them, 889 had keratoconus, 219 had forme fruste keratoconus, and 3829 eyes were normal according to Pentacam Keratoconus Scale. We developed 2 new diagnostic indices named KC-Score-1 and KC-Score-2 using backward elimination logistic regression based on only the pachymetry of pupil (PP) and pachymetry of minimum point (PM) parameters. The AUC of each index for diagnosing keratoconus were evaluated on new sets of data.

Results:

Our results showed that the magnitude of central corneal thickness parameters [pachymetry of apex (PA), PP, and PM] were lower and the value of (PA – PM), (PP – PM), KC-Score-1, and KC-Score-2 indices were higher in keratoconic eyes compared with the forme fruste keratoconus or normal eyes. Among all these indices, KC-Score-1 and KC-Score-2 had the highest and similar values of AUC for differentiating keratoconic eyes from normal eyes (0.98±0.01), and also for differentiating forme fruste keratoconus from normal eyes (0.88±0.01).

Conclusions:

The new center-wise pachymetric indices can help to differentiate forme fruste keratoconus eyes from normal patients. Among others, they might be used as additional Pentacam Keratoconus indices to enhance the final accuracy of the device for diagnosing suspicious cases of keratoconus.
