Rothia dentocariosa corneal infection in a patient with graft vs host disease

Abstract Details

Purpose:

To report a case of corneal infection from the rare pathogen rothia dentocariosa in a patient with graft versus host disease.

Setting:

Department of Ophthalmology, St. Thomas’ Hospital London.

Methods:

A 46 year old male with previous bone marrow transplant and subsequent graft versus host disease with ocular involvment presented in the eye emergency department with a 2 day history of vision deterioration, redness and pain in the left eye. Patient was prescribed elsewhere monthly bandage soft contact lenses to help alleviate the symptoms of the ocular surface disease related to the graft versus host disease. He was on regular treatment with topical lubricants.  On examination 3 superior corneal infiltrates were discovered, with underlying corneal edema and anterior chamber reaction. Scrapes were taken and treatment with hourly moxifloxacin was commenced.

Results:

he pathogen Rothia Dentocariosa sensitive to cefuroxime and gentamycin was identified after 3 days of incubation as well as growth of Streptococcus Viridans and treatment was altered accordingly with slow but good response.

Conclusions:

The rare pathogen rothia dentocariosa can cause a corneal infection in immunosuppressed patients that can be successfully managed with the combined treatment of cefuroxime and fortified gentamycin.
