Risk factors, clinical manifestations and prognosis of bacterial keratitis in western middle provinces of South Korea

Purpose:
To investigate the risk factors, clinical manifestations and prognosis in patients with bacterial keratitis whose causative organism was isolated in the region of Daejeon and Chungcheongnamdo, South Korea.

retrospective
Methods:
A total of 106 eyes in 106 patients with infectious keratitis, who were diagnosed by smears and cultures from January 2000 to December 2018 were retrospectively reviewed. We divided the patients into 2 subgroups by the age of 60 and analyzed the antibiotic susceptibility before and after 2011.
Results:
The mean age was 34 and 75 years in younger and older group, respectively. The most common etiologic event was contact lens wear (43%) in younger group and foreign body and ocular trauma (23%) in older group. The prognosis was poorer and the need of surgical intervention (67.2%) and incidence of evisceration (15.6%) was higher in older group than in younger group. The most prevalent causative organism was Serratia marcescens (20%) in younger group and Pseudomonas species (18%) in older group. The antibiotic resistance to quinolone was 14% and 25% before and after 2011, respectively.
Conclusions:
The regional difference was suspected concerning risk factors and causative organisms in South Korea. Caution is needed to select the appropriate antibiotics to treat bacterial keratitis because the antibiotic resistance to quinolone was increasing.
