Abstract Details
Purpose:
To report the case of a patient with a late-onset corneal infection after Descemet’s stripping automated endothelial keratoplasty (DSAEK).
Setting:
Cornea and Ocular Surface Unit. Vall d'Hebron Hospital. Barcelona. Spain.
Methods:
High resolution anterior segment optical coherence tomography (HR-AS-OCT) and biomicroscopy imaging of the onset, evolution and resolution, with medical therapy alone, of a bacterial keratitis in an eye with bullous keratopathy due to a failed lamellar graft in an eye with Fuchs heterochromic iridocyclitis.
Results:
As seen by HR-AS-OCT monitoring of the ongoing process, the infection affected only the host cornea. Microbiological cultures from corneal abscess identified Streptococcus pneumonieae. After 8 weeks of medical therapy alone, there was complete resolution of the infection. Visual acuity was light perception because of a severe leucoma.
Conclusions:
Late onset microbial keratitis after DSAEK is a complication rarely described in the literature. The infection may affect the receptor's cornea alone or may invade the graft. HR-AS-OCT may help in providing the involvement of the graft thus facilitating the decision of whether to add a surgical therapy to the medical one.
