Corneal keloids: description of three clinical cases

Abstract Details

Purpose:

To report patients with  corneal keloids

Setting:

Hospital General Universitario Gregorio Marañón

Methods:

Retrospective review of diagnosed corneal keloids in our Center. Epidemiological data, data from anterior segment  and ocular surface exploration with slit lamp, OCT of the anterior segment  ( OCTA; Cirrus, Zeiss) and corneal densitometry with topography (Pentacam, Oculus) were analyzed and comparing with healthy patients of the same age and sex.

Results:

Three clinical cases of corneal keloids are presented. All cases are female with pathology since childhood. Systemic personal history included skin keloids and rheumatoid arthritis. Ophthalmological antecedents included upper limbic keratoconjunctivitis and recurrent corneal ulcers. In the topography they presented astigmatism against the rule of 5.96 ± 4.08 with increase of the superior curvature. The corneal densitometric study showed significant differences with respect to healthy patients in all layers of the cornea. The OCTA images reveal hyperreflective images in the anterior stroma in a diffuse manner. The management of all patients was performed conservatively with scleral contact lens.

Conclusions:

In the diagnosis of mainly peripheral corneal keloids, corneal densitometry with topography can be very useful. Due to the poor response to the surgical treatment described, conservative management should be considered in non-aggressive cases.
