Intraocular pinhole implantation and DMEK in failed irregular penetrating keratoplasty

Abstract Details
Purpose: 
To report the outcomes of the use of an intraocular pinhole associated with DMEK to treat a failed irregular penetrating corneal graft with irregular astigmatism associated with a dense cataract. 
Setting: 
 Cançado Trindade Eye Institute 
Methods: 
 The left eye of a 63 year-old woman had a failing penetrating graft and dense white cataract limiting vision to counting fingers. Surgery included phacoemulsification with implantation of an intraocular pinhole followed by DMEK endothelial graft. A subtotal graft detachment was noticed in the initial postoperative period and graft re-bubble was carried out 1 week after initial procedure. 
Results: 
 Graft attachment was achieved after rebubbling with corneal clearing. The intraocular pinhole remained stable and well centered. By three months, visual acuity improved from counting fingers to 20/40 and J2 uncorrected. 
Conclusions: 
 Intraocular pinhole implantation can improve irregular corneal astigmatism. It may expand the possibility of treating a failed penetrating graft with endothelial keratoplasty.
