Purpose:
We would like to propose a novel sutureless scleral fixation technique, named the four-flanged technique, inspired by Lewis and based on Yamane’s flanged technique, using a 5-0 polypropylene suture, in order to simplify and improve the safety of scleral fixation using single-piece non-foldable IOLs.
Setting:
Centro Oftalmológico de Minas Gerais.
Methods:
Case series of five eyes from five patients, that evaluated a novel sutureless intrascleral fixation technique. The surgeries were performed from the same surgeon. All patients were followed by 3 months.
Results:
By the time of this article this new technique was performed in 3 patients, which became aphakic after phacoemulsification complications. There was no surgical intercurrence, and after 3 month of follow-up, all patients presented IOL stability and 20/40 or better visual acuity. No surgical complications were found.
Conclusions:
We believe that this new technique simplifies, improves, and quickens the single-piece non-foldable IOL fixations. Unlike the Lewis technique, this approach does not require scleral flaps or knots. Furthermore, this technique overcomes the issues concerning the insertion of the trailing haptic on the Yamane’s technique as the 5-0 monofilament has a totally adjustable length. It also eliminates the possibility of inadvertent trauma to the peripheral retina by the first needle which is left unsecured while performing maneuvers with the second needle. However, it is a new technique, futher studies are necessary with a greater number of cases and follow-up.
