Clinical results of femto-assisted phacoemulsification combined with non-penetrating deep sclerectomy with collagen antiglaucomatous drainage xenoplast in patients with coexisting cataract and glaucoma

Abstract Details

Purpose:

To analyze clinical and functional results of femto-assisted phacoemulsification combined with non-penetrating deep sclerectomy (NPDS)  with Xenoplast drainage implantation in patients with  co-existing cataract and glaucoma.

Setting:

Eye Center «East Sight Recover», Moscow, Russia

Methods:

The procedure was performed in 75 patients (72 eyes) 64-86 years of age with different stages of  POAG. In all cases the following examinations were performed: refraction, visual acuity, computer perimetry, optic disk retinal tomography, corneal-compensated pressure (IOP CC), pressure, equal to Goldman IOP (IOP G) and corneal hysteresis (CH) with the help of ORA device (Reichert). All examinations were performed before operation and 1,3,6 months, 1,2,3,4,5 years after operation. Postoperatively IOL position,  posterior capsule opacification (PCO)  were estimated. IOL EnVista (27), Hoya (29), Benz  (19) were implanted.

Results:

Mean preoperative IOP was 24.3±2.1 (mm Hg). By the end of 5 year the level of normalized IOP without hypotensive medication was in 40.9% of all cases. 58.1% patients had to use 1.7±0.8 medications. Stabilization of glaucoma process according to perimetry and structural parameters was in 34 patients, 35 eyes (77.8%, p=0.001). The best-corrected visual acuity 20/25 or better was achieved  in  78% , 20/40 - in 22 %. Posterior capsule opacification (PCO)  was found  in 9,6 % cases  in  18-36  months   after surgery.  There  were  no  cases of  IOL decentration.

Conclusions:

These results show that femto-assisted phacoemulsification combined with NPDS and Xenoplast drainage implantation is statistically effective method of  IOP normalization and stabilization of glaucoma process with decreasing of medication load compared with pre-operation level.  This procedure   provides an effective restoration of  visual functions and high level of patient satisfaction.
