Piggyback intraocular lens for extreme hyperopia after clear lens extraction

Abstract Details

Purpose:

To assess the refractive outcome of clear lens extraction (CLE) combined with piggyback intraocular lens implantation of patients with extreme hyperopia (+16.50 to +18.50D) and amblyopia.

Setting:

Euromedica Geniki Kliniki Thessaloniki, Greece.

Methods:

Two cases of bilateral high hyperopia and axial length <16mm (4 eyes) treated by piggyback implantation after CLE. The power for Intra Ocular Lenses (IOLs) was calculated with HolladayII / HofferQ and the target was emmetropia. Postoperatively follow up was for 4 and 1 year respectively.

Results:

Best Spectacle Corrected Visual Acuity (BSCVA) was improved in all eyes (1 to 3 Snellen lines). Postoperatively Uncorrected Visual Acuity (UVA) was the same or better compared to the preoperative BSCVA. Despite the residual refractive error (2 to 4 diopters) patients’ states very satisfied with the final outcome. No intraoperative or postoperative complication occurred.

Conclusions:

Formulae type and intraocular lens selection is very challenging in high ametropia. Refractive outcomes after CLE even in extreme hyperopia are very satisfactory. Operation of the non-dominance eye first and / or two stages procedure in each eye can improve the final results.
