Descemet's membrane endothelial keratoplasty (DMEK) and lensectomy with diffractive introcular lenses (IOL)

Abstract Details

Purpose:

To evaluate the functional results of the combined surgery of DMEK and lensectomy with diffractive IOL implantation in a population with Fuchs Endothelial Dystrophy and Cataract.

Methods:

This retrospective study included 29 eyes from 26 patients who underwent combined surgery of DMEK and lensectomy with implantation of diffractive AcrySof ReSTOR +2.50D Intraocular lenses (IOLs) (Alcon, Fort Worth, CA). Uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA), residual refraction, endothelial cell density and corneal pachymetry were assessed preoperatively and 6 months after each procedure. The surgery was always assisted by femtosecond laser to perform the rhexis.

Results:

Six months postoperatively, the mean UDVA and CDVA improvement from 0.30±0.17 to 0.47±0.27 and from 0.41±0.24 to 0.78±0.19, respectively. The mean sphere changed from -0.14±2.50 to +0.51± 0.87 and the mean cylinder changed from -1.42±1.44 to -1.40±0.99. The mean efficacy and safety indexes were 1.14 and 1.90 respectively. 89,65% of eyes gained at least one line of vision. 74,07% of eyes were within ±1.00 diopters of sphere of the target refraction, and 40,74% where within ±1.00 diopters of cylinder. Near VA in patients with CDVA = 1.00 ranged from 0.4 to 0.8 (decimal Snellen).

Conclusions:

Functional results of the combined surgery of DMEK and lensectomy with diffractive IOL implantation provided good visual and refractive outcomes in patients with Fuch’s distrophy, high anisometropia and significative refraction. Though the predictability of the sphere should be improved, the improvement on both UDVA and CDVA are good and the predictability of the cylinder is acceptable. Efficacy and safety indexes are very good and so the gain lines of vision.
