How to capture phacoemulsification with advanced corneal opacification

Abstract Details
Purpose:
Cataract surgery with severe corneal opacification is difficult to perform and predict postoperative outcome. We experienced that three good postoperative results performing phacoemulsification (PEA) with advanced corneal opacities.
Setting:
Department of Ophthalmology, Dokkyo Medical University, Tochigi, Japan.
Methods:
A 50-year-old male had corneal opacifications induced by phenothiazine-type antipsychotic medication. After capsulotomy with trypan blue staining, PEA was performed using pre-chopping technique.
A 70-year-old female had corneal opacifications induced by syphilis infection. After capsulotomy with trypan blue staining, PEA was performed using pre-chopping technique under chandelier lightning.
A 83-year-old female had unexplained sever corneal opacifications. After capsulotomy with trypan blue staining, PEA was performed using pre-chopping technique under chandelier lightning.
Results:
All cases improve visual functions and complain. However, the ratios of corneal endothelial cell loss (right and left eye) were 7.0 and 7.0% in case 1, 6.8 and 18.2% in case 2 and 48.0 and 25.9% in case 3.
Conclusions:
Even in cataract with advanced corneal opacification, it was possible to safely carry out PEA by devising the surgical procedure. However, due to poor visibility and difficult surgical procedures, the loss of corneal endothelial cells tends to be higher than in normal cases.
