Abstract Details

Purpose:

Aniridia Intra ocular lens (IOL) are used to correct iris deficiency in order to avoid photophobia and esthetic damage. Three colors of aniridia IOL HMK ANI 2 (OPHTEC) are available: brown, green and blue.

Setting:

We describe the first case of discoloration of a blue HMK ANI 2 IOL.

Methods:

A 38-year-old man underwent insertion of a +21 dioptre aniridia IOL with a blue iris for cataract treatment. One week later, he presented with xanthopsia and visual acuity of 0,8/10 Jaeger 28. His eye was red and painful, vanishing pigment was observed from the IOL iris (yellow instead of blue).

Results:

Anterior chamber irrigation and cleaning revealed yellowish aqueous humor and high levels of metals were detected using plasma mass spectrometry.  Inflammation regressed with steroid treatment and visual acuity increased to 6/10 Jaeger 3 but a few weeks later, the eye remained red and the IOL was completely white instead of blue.

Conclusions:

Principal complication of the type of IOL are hyphema and intra ocular pressure elevation, post-operative sterile inflammation are benign. There is no other case of discolouration with aniridia IOL. We still have no idea why the IOL discolouration occurred in our patient.
