Outcomes of phacoemulsification and intraocular lens implantation in radiation-induced cataracts of treated retinoblastoma patients

Abstract Details
Purpose:
To access safety, visual outcomes and complications of phacoemulsification with posterior chamber intraocular lens implantation (PCIOL) in eyes with radiation induced cataracts of treated retinoblastoma patients.
Setting:
Tertiary eye care centre, Advanced Eye Centre, PGIMER, Chandigarh, India
Methods:
Retrospective data analysis of patients registered between 1996 to 2016 was done. Patient who underwent phacoemulsification with PCIOL for radiation induced cataract in retinoblastoma treated eyes were included, any intra-operative and post-operative complications along with long term visual outcomes in terms of BCVA were noted.
Results:
Records of 910 registered RB patients revealed 52 eyes with cataract. Twelve eyes of 12 patients were included. Mean age at cataract surgery was 12.85±10.52 years ranging from 4 to 39 years (with median age of 11). Six patients underwent primary posterior capsulotomy (PPC) which was found safe and none of the patient required secondary intervention in these eyes. Visual acuity increased from a mean value of 2.8 +/- 1.1 to 1.1 +/- 0.4 on Logmar (p=0.002) at last follow up. Factors affecting visual outcomes were direct involvement of macula, radiation induced retinopathy or optic neuropathy and ocular surface problems.
Conclusions:
Modern technique of phacoemulsification, primary posterior capsulotomy, anterior vitrectomy with PCIOL is safe, if good preoperative control of primary disease is achieved. Long term outcomes are limited by primary involvement of macula by tumor, radiation induced retinopathy, optic neuropathy and ocular surface problems.
