True exfoliation syndrome (TEX) case in a housewife

Abstract Details

Purpose:

The aim of this report is to describe a case of a TEX  in a caucasian female.

Setting:

89-year -old caucasian female  was sent to our clinic  to be studied for  her unilateral left eye´s (OS) protruding detached delaminated  capsular flaps. She had a bilateral nuclear cataract and no personal glaucoma records.Unilateral grade 3 TEX diagnosis was made. There was no lens subluxation in the left eye.

Methods:

There was no lens subluxation nor donesis in the left eye. As her best corrected visual acuity (BCVA) was  20/60 OS, cataract surgery was offered. An uneventful cataract surgery was performed in her OS with the only particularity of tripan blue staining of the anterior capsule. The material obtained from the anterior capsule rexis was sent to the  pathologist  for analysis. Her  post operated BCVA  OS was 20/20 and  contact intraocular pressure (IOP): 19 mmHg.

Results:

The anatomo-pathological  results confirmed the diagnosis of TEX. (BMU and AP images will be provided).

Conclusions:

A unilateral  case of grade 3-TEX  in a  housewife is confirmed. TEX is an extremely rare condition. Though clasically reported to appear in workers related to  furnace factories (heat -related), nowadays the idiopathic cause seems to be the most prevalent. We include the description of the BMU and AP findings of TEX as well as a severity classification which is important for the cataract surgery planification.
