Choroidal evaluation in patients under alpha-lytic therapy

Purpose:
to check eventual choroidal changes in patients with major risk of Intraoperative Floppy Iris Syndrome (IFIS), assuming that the changes induced by alpha-lytic drugs could not solely be limited to the iris, but could involve all the uveal tract.

University Eye Clinic, Department of Medicine, Surgery and Dentistry, “Scuola Medica Salernitana”, University of Salerno
Methods:
21 eyes of 21 patients with age range from 60 to 88 years (77 ± 7 years) in alpha-lytic therapy scheduled for cataract surgery in the fellow eyes were included in the study.
All patients underwent EDI-OCT with Heidelberg Spectralis OCT at the first pre-operative visit and approximately one month after suspension of alpha-lytic therapy. The data were tested with the signed Wilcoxon rank test.
Results:
The mean choroidal thickness at the subfoveal level was 229 ± 82.6 µm in the first pre-operative visit and 217 ± 84.3 µm one month after suspension, while 1.5 mm nasally from the fovea it was respectively 205 ± 88.8 µm and 202 ± 84.8 µm and 1.5 mm temporally from the fovea was 221 ± 59.4 µm and 204 ± 58.2 µm respectively.
Conclusions:
Measurements showed a statistically significant reduction in choroidal thickness
one month after therapy discontinuation (p = 0.05). The decrease in choroidal thickness could be an important sign in deciding the timing for cataract surgery in these patients
