Cataract surgery after radial keratotomy with 16 incisions

Abstract Details
Purpose:
To present a difficult cataract case due to a previous radial keratotomy with 16 incisions. We present our approach for calculating IOL power and performing phaco surgery.
Setting:
Instituto Oftalmologico Hoyos. Barcelona (Spain)
Methods:
: 60-year-old female patient with 16 radial incisions and cataracts. Her refractive history was unknown. Refraction was +4,50 -3,00x90º in the right eye (RE) and +5,50 -2,50x110º in the left eye (LE) with a corrected vision of 20/30. We calculated IOL power using the flattest K reading on the center of the topography and SRK-T formula. We implanted acrysoft toric IOL both eyes. We performed a 2.2 mm clear corneal, selecting the widest area between two radial incisions. One radial incision opened up during the phaco maneuvers and needed stitching. No more surgical complications occurred.
Results:
One month after surgery uncorrected vision was 20/20 in the RE and 20/25 in the LE. Final refraction was plano in the RE and +1 -1 x 160º in the LE.
Conclusions:
Cataract surgery after radial keratotomy continues to be an unsolved challenge. To know about extreme cases with good results such as we have presented can be helpful.
