Traumatic rupture of polar posterior paediatric cataract

Abstract Details
Purpose:
Demonstrate total crystalline progressive opacification by rupture of the posterior capsular leaflet in a child with polar cataract.
Setting:
All surgeries were performed in the Sadalla Amin Ghanem eye hospital - Joinville - Santa Catarina - Brazil
Methods:
The child was followed up at this service due to unilateral posterior polar cataract for 6 years and at age 9 suffered a high energy blunt trauma. At 5 months, there was worsening of visual acuity for lumps and increased crystalline volume. The ultrasound showed echoes of high reflectivity in the anterior vitreous. Optical coherence tomography confirmed the posterior rupture of the capsule with crystalline hydration.
Results:
Due to the decrease in visual acuity and large volume of the lens, we opted for surgical treatment. The great difficulty was not cause the lens dislocation to the vitreous and to keep the posterior capsule as intact as possible. The IOL used was of three parts with wings in the iriano sulcus and body buttoned in the posterior capsule. In one year post surgery there is excellent centralization of IOL, normal IOP and corrected acuity of 20/80, retina at optimum.
Conclusions:
The polar posterior cataract is a challenge naturally. Blunt trauma can lead to cracks in the edge of the opacity and allow liquid entry of the berger space, which brings a volume increase and renders the lens white. The use of OCT in these cases helps an earlier diagnosis and allows treatment before total opacity.
