Abstract Details
Purpose:
To evaluate the outcomes of cataract surgery in pediatic uveitis
Setting:
Dr Rajendra Prasad Center for Ophthalmic Science, AIIMS, New Delhi, India
Methods:
It was an ambispective institution based case series. Patients<16 years with chronic uveitis who underwent cataract surgery from January 2005 to June 2018 after adequate control of inflammation were included. Etiology of uveitis, best corrected visual acuity (BCVA), postoperative complications, need for secondary intervention and immunosuppressants(IS) use were recorded.
Results:
98 eyes of 56 children with mean age 7.5±4.3years were included with mean follow up of 3 years. 55.1% had anterior uveitis, 28.5% intermediate uveitis, 2% posterior uveitis and 14% panuveitis. Etiology of uveitis was juvenile idiopathic arthritis(JIA)(55.1%), pars planitis(28.5%), tuberculosis (6.1%), sarcoidosis(2%), toxoplasmosis(2%) and tubulointerstitial nephritis and uveitis(2%). The final BCVA was better than 20/30 in 61.2%(22.2% JIA vs 86.3% non-JIA). IOL was placed in 81.4% JIA vs 100% non-JIA children. Complications included visual axis opacification in 18.3%, glaucoma in 25.5%, retinal problems in 35.7% and amblyopia in 21.4%. The mean use of steroids(>5mg)/IS was 2.5months post-surgery.
Conclusions:
Cataract surgery improved the visual outcome in children with chronic uveitis with optimum control of intraocular inflammation.
Non-JIA children have significantly better visual outcome than JIA group.
Holistic approach with treatment of retinal problems, glaucoma and amblyopia management can help in achieving good visual outcome.
