Congenital cataracts: surgical treatment

Abstract Details
Purpose:
To analyse the effects of the surgical treatment of congenital cataracts in correlation with the age of the patients, the timing of the intervention, type of the cataract and visual outcome.
Setting:
Alcon Infinity, Sistina oftalmologija, Skopje, R. Macedonia
Methods:
In a period of 6 years 28 children (38 eyes) undergone surgery, aged from 4 months to 11 years, 18 boys and 10 girls, 9 binocular and 19 monocular cataracts.
Preoperatively: funds examination in mydriasis, iop, slit lamp examination, ultrasonography, biometry, pediatric examination.
Deep anesthesia by a pediatric anesthesiologist.
Results:
In 9 children with bilateral pseudophakia the visual acuity is 0,50 and better. In 13 children with monocular pseudophakia the visual acuity is 0,40. 6 children younger than 2 years are not cooperative.
Postoperative glaucoma appears at two and early postoperative uveitis at 2 children.
Amblyopia management: correction of the refractive error, autorefractometry, sciacopy, close adjustment +3 D, occlusion.
Conclusions:
Early diagnosis and early surgical treatment of the congenital cataracts is most important for the normal visual development.
The appropriate surgical technique, iol implantation and orthoptic treatment throughout childhood in cooperation with the parents is most important for the amblyopia management.
